Having experimented with various devices over the past three years, I conclude that any ball trap device provides some protection against pull-out, though not total security unless the ring is tight enough to potentially restrict blood flow. Among all the anti-pullout options I’ve encountered, those that compress either the sides or top of the shaft appear to be the safest in my view. Personally, I find pin-type devices risky.
Do compression types work well? Have there been any skin issues linked to using them?

It is available in three sizes; only the smallest fit properly...and then turned out nearly impossible to take off (without breaking it) even after unlocking. In certain cases, a sufficiently small ring can work effectively without causing circulation problems.
I’ve noticed several cages on Fetlife that feature a U-shaped testicle separator located at the bottom of the ring and extending upward into the cage. It seems this design would go a long way toward making a device hard to escape because the testes are almost fully enclosed. Does anyone have experience with this kind of cage?

If the gap between the A-ring and the cage is small enough, you can't escape a ball trap device without damaging "something". My gap is 3mm and there’s no way I can get out. There's a huge difference between being able to pull back slightly versus completely escaping!! MsM’s ld
I tried one of the metal grip attachments that pinch the sides of the shaft closed. It successfully prevented pull-out but caused painful bruising from attempts at nocturnal erections, so I stopped using it.
I’ve been on what feels like an endless search for the same thing (an effective pull-out preventer). It seems it could be more of a sensor that notifies the keyholder via an app (think Fitbit-style)… or something similar to many E toys available now, though I haven't found one that quite works yet ☹️ The other idea is more mechanical: a device that closes after pull-out. Yet again, I haven’t found a concept there that quite works. Ahh the search continues
Regarding Holdingout1's comment...I have seen a description of a "pull-out notification device" which simply consists of a string looped around the shaft before it goes into the cage, with the ends extending outside through a small hole. The ends are then placed into a small clear plastic tube bearing the KH’s signature and superglued shut to create a waterproof and tamper-proof capsule. This does not prevent pull-out but would alert the KH if one happens successfully, as reattachment while the cage is in place would likely be impossible. Of course, we must also consider hygiene issues and debris getting trapped by the string. I’ve also considered using a silicone ring with lead to achieve the same result, which might be easier to keep clean. Just some random thoughts.
Some ball trap cages come equipped with a steel urethral tube that extends behind the base ring. This doesn't guarantee you won't pull out, especially if the base ring can slide down. However, you likely won’t be able to re-enter that tube without unlocking—particularly if your cage has a close-fitting inner diameter.
