My testicles are compressed and twisted inside the cage, resembling part of a balloon sculpture. They’re pushed forward enough that I have to walk with an altered gait to keep from hurting. Is this due to a poor fit? Can my balls hang more naturally, or is this just how things will be going forward?

Discomfort is possible but shouldn't hurt. You didn’t specify which device you’re using, and they all fit slightly differently. The base ring needs to be snug without being too tight; you should still be able to slide the tip of your finger between it and your body. The tube length ought to be a bit shorter than your flaccid penis. A properly sized tube helps "curb" erections, which hopefully reduces discomfort when the tube shifts away from your body, pulling the base ring and testicles with it. It appears that men who are positioned "high and tight" sometimes struggle more to find the right device.

Certain cages give me trouble. I believe it comes down to the base ring size. Smaller rings tend to push everything outward more. I balance a larger base ring with a smaller gap. If you experiment with that combination, it might resolve your issue.

To wear a device featuring a ball ring successfully, two elements need to be correct: 1) The diameter of the ring. 2) The gap between the ring and the tube or cage. If your balls are perpetually tight, you may need to stretch out your scrotum for comfortable wear. It shouldn't hurt constantly; most of the time, it should go unnoticed.
