Has anyone worn a spiked anti-pullout ring like for an extended period? I find it somewhat intimidating.

I owned a cage with spikes included, though they weren't my main focus. The points were quite sharp and could easily pierce the skin; the center one rests under the urethra, so over time it might cause some PA pain away from the glans. I tested them for a few hours before reaching my limit—there was noticeable discomfort when aroused. They probably work long-term if you file or use a Dremel to round off the spike tips. Just not quite right for me, so I didn't keep using them.
