I’ve had the CB4000 in “The Curve” for a while, and I’m using the second smallest ring. You barely notice the smallest spikes until you get erect; then they make you squirm slightly, but once soft again, it's very pleasant since they double as spacers and give more breathing room. I mainly wear them because without the spikes my cock shifts around too much in the cage. I've been wearing my spikes for about 7 days now. They do leave a mark if you get hard frequently, but mostly when you remove the device to shower and put it back on, the mark fades quickly. So, 24/7 use is easily achievable. However, those bigger spikes are quite painful, confirming they're definitely made for punishment.
